| If you were a student of RES, print the Form, fill out your information, and fax to RES at 615-333-0171. RES will fill out the rest of the Form and mail to your home address on file. You will then mail the Form to the Medical Board. If you were not a student of RES, you must contact your Course Provider. If you did not have class hours in the full spine, as defined in the new x-ray rules, you cannot upgrade your license to full spine. |
